Primary Care

About Primary Care (Family Care)

At Grace and Gratitude Wellness, we go beyond traditional healthcare to provide a personalized, root-cause approach to your well-being. Our primary care process is designed to uncover the underlying factors affecting your health and create a tailored plan to help you thrive.

Your journey starts with an in-depth consultation, where we take the time to understand your medical history, symptoms, and health goals. From there, we develop a customized diagnostic and treatment plan that may include traditional medicine, advanced testing, innovative therapies, and lifestyle recommendations.

Our commitment doesn’t end with a single visit—we provide ongoing support, progress evaluations, and wellness maintenance to ensure long-term health and vitality. At every step, we partner with you to achieve sustainable wellness.

Our Primary Care Process

1. Initial Consultation

Your journey begins with an in-depth consultation. This is our opportunity to learn about you, your health history, and your current concerns. During this visit, we’ll discuss:

  • Past medical and surgical history
  • Medications and allergies
  • Current symptoms and health goals

We take the time to understand the whole picture so we can create a plan tailored to your unique needs.

2. Personalized Diagnostic Plan

After your consultation, we collaborate with you to design a diagnostic plan aimed at uncovering the root causes of your health concerns. This may include:

  • Traditional labs, imaging, and diagnostic tests
  • Advanced testing options like comprehensive stool analysis, hormone panels, micronutrient assessments, heavy metal screening, or food sensitivity tests

Each diagnostic tool is chosen based on your specific symptoms and health goals.

3. Customized Treatment Plan

Once we’ve gathered the necessary information, we’ll create a personalized treatment plan. This may combine:

  • Traditional medical interventions
  • Innovative therapies like halotherapy, infrared sauna, cold plunge therapy, and IV treatments
  • Lifestyle recommendations, including nutrition, exercise, and stress management

Our goal is to provide a holistic approach that addresses the root causes of your concerns while supporting your overall well-being.

4. Ongoing Care and Support

Your health journey doesn’t end with a single visit. We’re here to support you every step of the way, providing follow-up care, progress evaluations, and adjustments to your treatment plan as needed. Our team works with you to ensure sustainable improvements and long-term wellness.

5. Wellness Maintenance

Once your initial concerns are addressed, we’ll guide you in maintaining your health through preventive care and regular wellness check-ins. By staying proactive, we can help you prevent future health issues and continue living your best life.

At Grace and Gratitude Wellness, our process is designed with one goal in mind: empowering you to achieve optimal health and well-being. Every step is intentional, collaborative, and focused on you.
